Class 2 OTC Drugs and Designated Class 2 OTC Drug products

These are OTC drugs, excluding Class 1 OTC Drugs, that may cause health problems severe enough to interfere with daily life due to side effects or other factors, and that are designated by the Minister of Health, Labour and Welfare. Among Class 2 OTC Drugs, those requiring particular caution depending on conditions such as drug interactions or patient background are distinguished as Designated Class 2 OTC Drug products.

Class 3 OTC Drug

These are OTC drugs other than Class 1 OTC Drugs and Class 2 OTC Drugs. Although their risk is relatively low, they are drugs that may cause changes in or disturbances to the body.

2. Explanation of Drug Labeling

The direct containers or direct packaging of specially designated OTC drugs and OTC drugs display the words “specially designated OTC drug,” “Class 1 OTC Drug,” “Class 2 OTC Drug,” or “Class 3 OTC Drug” enclosed in a frame according to their classification. Designated Class 2 OTC Drug products display the character “2” enclosed in a circle or square. If the display on the direct container or direct packaging cannot be seen from outside, it is also displayed on the outer container or outer packaging.

6. System Concerning the Sale of Designated Drugs Subject to Abuse Prevention Measures

Definition of designated drugs subject to abuse prevention measures

Designated drugs subject to abuse prevention measures are pharmacy-manufactured and marketed medicines, prescription medicines requiring guidance, or OTC drugs that the Minister of Health, Labour and Welfare designates, after hearing the opinion of the Pharmaceutical Affairs Council, as medicines that may cause excitation or suppression of the central nervous system or hallucinations when abused and for which abuse prevention measures are necessary.

As of May 1, 2026, the designated ingredients are ephedrine, codeine, dihydrocodeine, diphenhydramine, dextromethorphan, pseudoephedrine, bromovalerylurea, and methylephedrine. Except for topical preparations, this includes hydrates and salts of each ingredient. Preparations whose primary active ingredients are crude drugs are excluded.

Labeling of designated drugs subject to abuse prevention measures

As a rule, the words “Confirmation Required” are displayed on the immediate container or immediate packaging of designated drugs subject to abuse prevention measures. For products whose contents exceed the quantity specified by the Minister of Health, Labour and Welfare, the character meaning “required” in “Confirmation Required” is enclosed in a circle or square. Due to transitional measures, products bearing the former labeling may continue to circulate for the time being.
